>personal appearance for 8 years old and below, 65 years old and older and for mentally and physically incapacitated
>3 colored photos of applicant w/ royal blue background.Applicant should be in decent attire w/ collar.Photo size: 4.5cmx 3.5 cm.Facial image size, not less than 3 cm.
>application form
>birth certificate in security paper issued by the national Statistics office
>proof of identity /supporting documents indicating full name, date, and place of birth and citizenship
>other supporting documents such as marriage contract, Certificate for first time Applicants(for those married to foreigners),death certificate of spouse if widowed,certified true copy and photocopy of the 1st page and the dispositive portion of judgment on annulment asnd NSO issued MC w/ annotation of the annulment decree

Note your reference number
Prepare all requirements and payment on the scheduled pick up date
Prepare for your personal appearance date at DFA
Your passport will be delivered to you after processing
